IF YOU DO NOT LET THEM KNOW THIS AND WE CALL THEM, AND THEY AREN'T EXPECTING OUR CALL, YOU WILL NOT BE CONSIDERED FOR ADOPTION. **DO NOT FILL OUT THIS ADOPTION APPLICATION UNLESS ALL FAMILY MEMBERS AGREE AND ARE 100% READY TO ADOPT A DOG. **DO NOT FILL OUT THIS APPLICATION IF YOU ARE CURRENTLY APPLYING TO ADOPT A DOG WITH ANOTHER ORGANIZATION. **DO NOT FILL OUT THIS APPLICATION IF YOU HAVE CURRENT PETS WHO ARE NOT UP TO DATE WITH THEIR VACCINATIONS. WE WILL CALL YOUR VET FOR REFERENCE AND IF THEY TELL US YOUR PET IS NOT CURRENT WITH VACCINES THEN WE CAN’T ADOPT A DOG TO YOU. **PLEASE ANSWER ALL QUESTIONS BELOW AND LEAVE NOTHING BLANK ON THIS APPLICATION OR YOU WON'T BE CONSIDERED FOR ADOPTION. The adoption application is utilized in the initial phase of the adoption process whereby the Fly To Freedom Dog Rescue collects information about you and learns about you, your home, your property, your work situation, your family members, your other pets (if you have any), your veterinarian doctor (or the one you will use) and your references, to determine if you will be a good match for adoption. The collection of this information also allows us to speak to your veterinary doctor (or the one you will use) and your references. Filling out and completing this application does not grant you automatic approval to adopt a puppy from the Fly To Freedom Dog Rescue. You are only granted approval when our Adoptions Director contacts you to notify you that you have been approved. Adoption fees for U.S. residents for a Fly to Freedom Dog Rescue puppy are:
- $625 for a spayed/neutered puppy
- $475 for a non-spayed/neutered puppy Most of our puppies are young, around 3 to 4 months old. Because they are so young they have not been spayed or neutered. If you are adopting a young puppy from our organization, you will be responsible for getting your dog spayed or neutered. You will pay us a $100 deposit. When you show proof of spay or neuter, we will return the $100 deposit to you. Every cent of your adoption fee goes directly toward the expenses incurred in preparing the dog for adoption including boarding fees, medicine, vaccines, de-wormer, flea and tick preventive, food, collar, leash, carry on crate, transport fees and airline ticket (yes, we have to pay for an airline ticket for each pup that flies into the U.S.). Please be sure that you are 100% willing and able to pay the adoption fee prior to filling out this adoption application. If you are selected and approved for adoption, we require full payment of the adoption fee before the dog is provided to you. The adoption fee cannot be broken into payments. If you work for an employer that has a corporate matching program and you approved to adopt a dog, please ask your employer to match your adoption donation. They can make their matching donation directly. Please provide them the Fly To Freedom Dog Rescue tax ID number 85-4190501. Once you provide all of the information we ask below, your response will be automatically sent to the Fly To Freedom Dog Rescue team for review. Once selected for adoption, our Adoptions Director will make phone calls to all your references. Once you have been approved to adopt, an adoption agreement will be emailed to you for your agreement and signature and payment. Please provide the names of personal references (non-family members) who truly know you and can speak about your home, property and how you are/would be as a dog owner. We will call these individuals to learn what type of pet owner you are/will be. We will also call your veterinarian with whom you have a relationship with, or one that you will go to if you are a first-time adopter, to understand what type of dog owner you are and if you are up-to date with vaccines on any current animals in your home. It’s CRUCIAL that let your veterinarian and personal references know to expect a call from the Fly To Freedom Dog Rescue to help us expedite your approval process. If you do not notify your veterinarian and we call and they are not expecting our call and can't provide us with a reference about you, then you will be automatically disqualified to adopt. Make 100% sure to call your veterinarian letting them know to expect a reference call. We kindly request that you be patient with our organization, as we go through a process of evaluating every applicant who applies for a dog and then contacting references. This can take time (up to one week), as our team is comprised of volunteers who have full time jobs. We will absolutely notify you whether or not you have been chosen for adoption. Please DO NOT hunt down our team members, send them a personal Facebook messenger message or Facebook friend request to inquire about adoption and your application.
